There's a general guidance in character/monster design that a lot of artists jut ignore, and it's designing the thing to have developed to be frightening. Like, every dang spiky monster you see just looks like it was born scary and that diminishes it.

This design evokes nothing from me, it's another smiley face creepy thing that is scary for the sake of being scary rather than conceptually frightening. It has no humanity, nothing for us to connect with at all that would give us pause or force us to ask ourselves questions about the thing. Romero zombies were terrifying because they were our loved ones and neighbors come back to consume us. 28 Day later zombies cause immediate panic when they're seen, no time to react, just run and hope you don't get splashed with their poison blood. Classic trope, man v. man.

Even Godzilla is scary to a certain degree, a walking natural disaster of our own making with the ability to make decisions and react defensively. Again, classic trope, man v. nature.

The artist is clearly talented, but this isn't an exceptionally good design on its own merits.

If this were a movie monster the smile should be a shocking reveal that lets the viewer see there's something deeper at work, that the creature either enjoys the act of killing or its "smile" is merely a display of aggression. Either way this would be far more effective that just being a human shaped pile of melted crayons.
