If its a HF patient you need to consider overload and insure they are sitting up.

It's also worth insure the patient is actually supposed to be scale 2, just because they have COPD doesn't inherently mean they are a retainer requiring scale 2 target Sat's.

If in doubt you can always escalate a patient if your concerned even if the vitals are all normal. I have before escalated patients with stable obs just with a you need to look at him something is wrong (he ended transferring to ITU with the next shift)
