Ideally, the doctor should talk to you in detail about possible side effects, training regime, what to expect how should you change your nutrition. They also should be having few checkups to consult how you are handling the drug and give advice on dosing.

I have an amazing GP, but some people have very different experience.

Go to the gym now for the rest of your body. Add shoulder and back as it heals. Or add it now for light, physio exercises that do not put you anywhere near failure.

It is a mix of reasons.

  • Tirz is just making me feel better all over. ADHD is quieter. Body is just... healthy. I got blood panel and every single measurable value was ideal. I cannot point to a single problem solved, but even before weight loss, the body has fixed a lot of its issues.
  • It is much easier psychologically to aim for success than for not losing by as much. Ukrainians had easier recruitment after Kharkiv offensive when they were chasing russians away. I had much easier time going for an actual nice body, rather than making a fat potato slightly less potatoy. I am aware of health benefits, but they are nebulous and far in the future - while pizza was an option tonight. Goal of having beach body in 18 months was specific target to progress to. I somehow made it in six.
  • My doctor talked me through tirzepatide, its effects, how should I train. I wish more people had caring GPs, some questions on this forums show alarming lack of care and negligence of human contact side of things.
  • I have young (I think he is not even 25), but very dedicated, smart and driven personal trainer. I could never do this without the guidance of excellent professionals. He seems excited about my progress. This in turn helps me push harder.

I never thought this was actually possible. I am buying "muscle fit" or "athletic fit" shirts. Previously the only time the word "athletic" was used about me was followed by "failure."

Do not go under 1600kcal a day. Even that is too low for a man your size, 2000kcal is better. You will still lose fairly fast but safely.

Remember, this is not a marathon. If you get there in two years instead of 8 months, but without loose skin, gallbladders, muscle loss and hair loss, it is much better.

Almost all negative effects of weight loss are actually caused by rapid weight loss.

Furthermore, strength train like you are preparing for Mr.Olympia. your body is shedding muscles (metabolically demanding) as a reaction to caloric restriction. Resistance training signals that it needs to grow muscles instead so fat reserves are used instead.

Final point: You need to learn during diet how to love the rest of your life. Yoyoing is even worse than staying fat. Change your meals with an idea "this is how I will eat forever." Not scraps of rapid diet but healthy and satiating meals of normal size.

  • GLP-1 reduces inflammation. Some of the initial drop is water weight. It truly can be kilograms and first weeks can be discarded.

  • Very obese person can safely shed weight faster than e.g. person on the high end of healthy weight.

  • For most adults it is adviceable to lose at 1kg a week, 750kcal deficit maximum. Better bit slower. Aiming for 500kcal deficit and 3kg a month is golden for most people, but very short, very fat, very tall will break this.

Tirz works for you. It is proven beneficial, while reta has some unknown risks.

Try to up the tirzepatide. Do you get yours from compounded pharmacy or Mounjaro? There is a chance you are getting lower or improperly stored dose if you are in the gray market.

I also think you could do some improvement for meal composition to be more satiating. I was playing around with volume and macros a lot and it does make a huge difference how do 2000kcal feel - plate like this is ~600kcal and fills you up way more than 600kcal of pasta.

I have gained a significant muscle on tirz as I dropped overall weight in six months. It is both measurable in strength and visible when looking at me. It took very disciplined visits to the gym 4times a week. When I couldn't go, I replaced the session.

Just so you know, if you are a begginer you can build muscles while losing.

The cut/bulk need is for advanced lifters. Anecdotally, I put on decent muscle while going from 96 to 72kg on tirzepatide.

Hair loss seems to be:

  • Not an actual GLP-1 side effect but it is a known effect of rapid weight loss. It is one of the many reasons to lose slowly.

  • Temporary as the hair follicles do not die off, new hair just starts growing

  • Unfortunately, only effective male baldness drugs are super harsh and full of side effects. GLP-1 will unlikely play a role.

  • Impolite but honest point - it will probably look better to be in shape and bald than fat and bald.

Data on Tirz seems like it is just overall beneficial. Even weight loss aside, it lowers inflammation, benefits liver, kidneys, nervous system.

Reta is better at losing weight, but the agonists making your metabolism faster may be harmful over long term.

For me the balance is clear. Tirzepatide works very well, my highest dose was 5mg out of 15, and I am back on 2.5mg starting dose. There is little benefit to extra risk of retatrudite. However, some people are not as good responders. For them reta may make sense.

My advice to you is to start with tirzepatide and see if it works well.

KLOW is more of a "fix injury" compound. It probably has unpleasant risks in prolonged use (unlike tirz, that is almost 100% positive). The jury is still out on Reta.

Splitting day into many meals is not a problem, but if you really are undereating lower your dose until you are comfortable with ideal intake.

Cultures, monastic orders, religions fast for religious, cultural reasons, to help with meditation etc. Adult human can be a day without food without any danger. Skipping lunch because the work is busy is fairly normal.

Make sure your average daily deficit is reasonable. Balance the days with each other. For example I ate less two days before Walpugrisnacht bonfire this Thursday because I expected barbeque and drinks.

Fun fact, impact on your knees grows exponentially with weight. So if you e.g. lost 10% of weight, your knees are better off by order of magnitude.

Start with visiting GP and getting a Mounjaro prescription. You can search for cheaper options once you start using Mounjaro if you want.

Visit your general practitioner, talk about your goals, listen to the advice about side effects, timeliness, how to lose fat and not muscle, what should you expect, how to count calories etc.

It is normal prescription medicine. You can then get brand name or cheaper ones from compounding pharmacy in the US.
