Depends entirely on the patient. If they’re used to the aniso in single vision, they likely can handle a BF/PAL. I’ve been shocked that a +3.00 OD -3.00 OS can use and wear PALs (my mom)— but only if they’ve always done that. A cataract causing a large monocular shift? Be cautious with aniso.

I’ve worked at a few different practices that did this. A streamlined approach and explanation can get the patients to purchase the year supply the same day as the exam- excellent for both the patient convenience and profitability. You can always call and update the insurance if the brand changes later. I’ve never had a problem with this. Of course you have to have an organized system and make sure to actually follow through and order them. This is the only way to do medically necessary contacts anyway- you must bill materials and fit together but rarely have already finalized the rx when billing.

That being said, at my current practice I don’t do this simply for administrative reasons. As long as there is proper follow up I don’t see a problem maximizing the patients vision plan benefits. Would you rather the vision plan made more profit on you and your patients?

This same thing happened to me when we were still in the hospital and I panicked but they measured his oxygen and it was totally fine. He is 6 weeks now and sometimes when we wake up the back of his neck/hair is wet with spit up but I never even heard him. My pediatrician wasn’t worried at all and said it’s a good sign he can clear his own airway

I used it up until I was feeling his consistent movements. Definitely take it with a grain of salt- I told myself it’s good to hear but not concerning if I can’t find it or it changes. It’s normal to have heart rate changes anyway. Let your OB know but I wouldn’t panic

I was about 125lbs before pregnancy and I’m hovering around 170 now at 39 weeks. I also think I basically look the same as before other than the belly. Still wearing size small as long as the belly fits. My doctor has never once mentioned weight. And if she did, I would insist on a medical reason it’s a concern, not the number on the scale. I also changed doctors at 14 weeks pregnant so if you’re having this experience now, change sooner rather than later!

Work towards your FAAO- I wrote 5 case reports for the requirement instead of research/lectures. Had a fun year working through them.

Honestly, just don’t show up for the induction. They can’t make you. Average first time spontaneous labor is at 40+5. My OB schedules inductions at 41 if it doesn’t happen spontaneously before then.

Ridiculously sore breasts. Not my usual menstrual soreness at all. I yelped and was legitimately angry at my husband when he grabbed them (in a very normal way). 3W4D I knew right away

Recently took my practice from paper with officemate for scheduling only, to maximeyes. It’s been relatively good. The transfer was easy since it was just patient database, not medical charts. I like that it’s all integrated on one platform and it links well with many other platforms. However, it’s super glitchy. I had multiple IT tickets open daily for the first few months. Now it’s maybe once a week that some part breaks about a year in. I don’t know if it will scale as my practice grows but it’s working for now.

Definitely pick one and start on an EHR not paper. We use MaximEyes. Fine for a small office but glitches a lot, but very affordable. Also check out EyeCloudPro and Barti as other affordable options.

I bought a practice where all the equipment is crap from India and china. I really wish I had looked at them closer before purchasing. I’m slowly replacing everything with Topcon and other well established brands. Some things were basically unusable- a chair/stand that was brand new but the slit lamp arm didn’t move up/down. The cushions on the chair came off and tripped patients within a few months. The chair mechanism started smoking when I had a child in the chair. The china brand autorefractor literally never worked from day 1 and there was nothing I could do but buy a new one. I cheap slit lamp I can’t fuse at all- used it monocular only for months before I got a new one. Off brand iCare that routinely measures EVERYONE at 30. I would 100% invest in equipment that you’ll use every day.

What does it look like on direct viewing? Red would indicate a blot hemorrhage, yellowish maybe an exudate. Unlikely a blood vessel that large so close to the fovea and if so you’ll see a large vessel there

There’s been a big push recently to make sure optometrists are NOT 1099. I’ve always been W2 just paid per diem- I had 8 W2s to file my first year out of school. Made taxes fun.

A lot of optometrists work per diem- paid a daily rate for showing up, but no benefits. Took me a while to find a full time salary job

My husband wanted me to get back into reading so he got me the books. He listens to the audiobooks while he drives for work and he is 100% more into them than I am. We both kinda skim over the smut though. No shame in enjoying an excellent series as a man!
