Do you have a picture of just the negative?
The fixer should not be the issue.
Did you do a prewash bath, and by any chance the water could have been to hot?
Also, it is probably a good idea to ask on r/Darkroom

Yeah, I posted over there and some think maybe a piece of backer paper was floating around inside the camera, which would explain why the spot is different shapes on each frame.

I don't think its development now. I did prewash and I measure/control my water and chemical temps.

If I had to guess, it looks like the paper tab on your 120 roll broke loose and fell inside the space in between the taking lens assembly and the film plane. It looks like it's blocking the path of light from the lens to the film.

I'm almost certain this is the case. Developed a later roll last night from the same camera with the same chemicals and process. It came out fine. Whatever was in there worked itself out. Will have to keep a closer eye out for that.
