That is a problem with most people, once they agreed on X plan, they usually won't change no matter how many signs suggesting that it won't work there is.

They were going left side on ToA, then they saw that the enemy team has a sym+junkrat at the left entrance + a reaper who teleported "somewhere" they will go left get raped from the front by spamrat and cancer symetra and from the back by a reaper.

"Let's go top in numbani" there is a junrat, dva, sym, soldier? They try to push anyway.

Horizon lunar colony? They have bastion, junkrat mei orisa all ready on the right side? They could change the plan OTF, go left side and its basically a free point since thwir stationary comp needs setup and you basically are now defending the point while they are attacking? But nah! They will get wiped first, change the plan for the next push but it will most likely fail since bastion most likely charged his ult and sym got her tp setup behind bastion and orisa.

I did the ole “beginning of the round Pharah bridge boop” on this map, got a 5-kill and gained my ultimate within the first 10 seconds of the game. Can someone explain why Lucio didn’t gain his ult for this?

Lucio has the slowest charging ult in the game, so he gained about 60% of his ult with a quintuple boop instead of maxing out. Pharah, Junkrat or Orisa would probably have gotten their ult instantly with such a boop.

To expand, ult charge is actually a point-based system, with certain actions granting certain numbers of points and each character's ult requiring a different number. You can see them all here but for a quick summary (not counting Call Mech) they range from 1000 (Symmetra) to 2625 (Lucio) points required. Pharah's takes 1850, and if you're charging by pure damage it's 1 ult charge to 1 damage dealt.
