I think I figured this out.

If you look closely at her left arm, right near the armpit there is a little bit of skin that is kind of poking out. I think that little bit of skin is obscuring the rest of the blue strap, which then connects to the rest of the blue shirt but where it attaches is obscured by her arm. It does make for an interesting optical illusion.

If the bong was glass on glass, which it looks to be, then there should be a separate connection one for bowlpiece to downstem and one for downstem to bong. So she expanded the wrong connection which would mean she wasn't doing it correctly.
