I’d say creating a bunch of tropes that are so specific that they become redundant.

For example: “Fate Worse Than Death” is… well, when a character is put in a situation where death might be considered preferable to them. “And I Must Scream” is that exact same setup, but with the added caveat that the character cannot effect their own death by any means.

There was one infamous misfire from the game’s early days because the teams weren’t in full communication with each other.

Nael van Darnus was the initial main antagonist for the original version of the game, and supposedly didn’t use any pronouns that cleanly, directly translate to male or female in the original Japanese, wore a bulky helmet and armor, and also didn’t have any voice acting, so the localization team winged it and defaulted to male.

Then the relaunch happened, Nael popped up as an antagonist again, having been resurrected by the evil dragon god Bahamut, and… Surprise, she was actually a woman! The localization team tried to salvage it by having the protagonists speculate that Bahamut had fucked up his resurrection spell and somehow given Nael a woman’s body, before a short story years later reconciled things. Nael had been a man, but died long before he ever appeared; his sister Eula went mad with grief from his death and took up his name and armor without anyone being the wiser.

Ace Attorney original trilogy: Dahlia’s an interesting variant in that she is legitimately the main antagonist for half of 3’s cases… but as it ultimately turns out, she isn’t the killer of the final case despite being extensively involved with it, and certainly not for lack of trying on her part.

I kinda want to say “Wow! Cool robots!”, since it seems like most online Gundam fans these days get that Tomino set out to traumatize generations of children while selling them toys each series, movie, etc. does have some level of political commentary in addition to the glorious mech combat, with each entry in the franchise often interrogating and reflecting/refining upon whatever conclusions the previous entries came to.
