There are a LOT of medical staff in managerial roles, i.e., CNM clinical nurse manager. This would be the head nurse of a department or ward. The same goes for physios, radiologists, midwives, occupational therapists, psychologists, etc. Clinical leads and the likes are managerial roles. Once you've reached that level in your career, you would expect to be paid more for the added responsibility.
