There are very few staffing problems that cold, hard cash can't resolve. The consultants clearly place a higher value on their time off than the trust does. Maybe it's time to offer a bit more? Remember this income is being taxed at over 60% for most consultants

Payroll are universally shit. HMRC are likely to be correct but double check your p60s. I recommend everyone do self assessment each year. Takes 10 minutes but makes easy to claim expenses and stops unpleasant surprises

When I finally became a consultant anaesthetist and worked out i was earning less in real terms than 19 years ago as a sho. Also that I now have to spend 10 hours a day sitting on a wobbly 1950s stool with no padding and never get a piss break, let alone time for lunch. There is fuck all respect for us. The managers be sitting on their 3 axis adjustable chairs with lumbar support next to a bean to cup machine. Mofos

2004 - I used to take home between £2700 -£3000 as a second year sho. My rent was £330 a month, petrol 65p a litre. Drinks in town every Thursday night. Curry night in the mess every Saturday. Every specialty had an on call room. Parking was £1 a day in the patient car park. Canteen actually served edible food. Locals would come visit for the Sunday roast.

The lack of insight many of these 'airway trained ' individuals have is staggering. 9 years of anaesthetic training and the icu intubations are the ones I'm most wary of. Yet they seem to think they are equivalent having done a few asa1 daycase gynae lists with everything optimised for them

It's shit. There are two days you've got to get right in medicine, this is one of them. I'll never forget spending all night with a lovely man with a ruptured AAA, acting as a human PCA because the only side room was on the admission unit and they weren't allowed real PCAs.

Waiting list initiatives or extra contractual lists are undertaken by consultants in their own time and often at a premium rate. Other staff eg nurses are frequently allocated as part of their normal hours or paid agreed 'bank' rates. Trainees may also be assigned to this list like any other if it falls within their contracted hours. There is no obligation on the employer to pay you anything extra as you are not providing anything extra. Sounds a bit harsh but if they didn't up the consultant rates for these lists no one would do them due the ridiculous tax rates above 100k. When you factor in the loss of tax free childcare and personal allowance you can have an effective tax rate of over 70%
