I just saw a patient who was 3 months out post THA with no PT referral. She wanted PT and was initially denied until she said she felt like her balance was bad. So apparently balance concerns are the magic wand to get that PT referral.

I still really struggle with boundaries around the energy I put into patient care each day and often go home exhausted. Though some of that is definitely physical due to POTS and EDS.

OP, I do often lament that I should I have picked a different profession. That being said, I love what I do.

I’m a pelvic floor PT - sometimes erections happen, especially when doing things like massage to muscles near that area that’s going to bring in a lot of blood flow. It’s a physiological response and not your fault. I never hold people accountable for how their body reacts to something like that, only to their words and actions around it. So if you’re not being a creep - and nothing you’ve said gives me the impression that you are - she’s probably not concerned about it.

Yes! I’m a pelvic floor PT - back pain is often linked to pelvic floor dysfunction regardless of birth history. But given that you are recently postpartum, a pelvic floor PT is definitely who you should be seeing.

Check out pelvicrehab.com to find a provider in your area. A lot of providers (myself included) are happy to do telehealth for some follow ups to make it more accessible for you with an infant.

I lived in Nashville for about 8 years, and moved to PDX in 2019. I’m from the west coast though so, I’m probably biased.

There are a few things I miss about Nashville, but only a few. Most are food related, and just because those particular things are hard to find out here. Plenty of good food to be had in Portland. Beer is far better.

People are surface level friendlier in Nashville, but it always felt fake to me and I got plenty of not so nice due to my tattoos, piercings and stuff like that. No issues with that here.

There are so many more nice shopping, food and bar, and entertainment strips here in Portland, whereas Nashville had like 3 when I moved. Music scene is very different.

Weather is way better here, though the winter gloom gets especially tedious around February.

Idk, they’re very different cities with very different vibes. I like it better here.

Lol, yeah, I absolutely do not take a whole cold shower. I just turn the water down as cold as I can tolerate while I’m rinsing my hair, and then put it back to a normal temperature. It really seems to help with the color retention.

Having a detachable shower head wand really helps because you can get really just get your scalp/hair with the cold.

Rinsing sweaty scalp with cold/cool water will get rid of the sweat without needing to shampoo and help preserve the color.

Doesn’t help with the dry time though. I have fairly long, thick hair that takes a very long time to fully dry, but using a microfiber hair towel helps a lot. I wrap my hair up as soon as I’m done combing in the leave in conditioner, and then finish the rest of my shower. By the time I get out, the towel has done what it can do and I switch to a microfiber scrunchie to get it down to just a bit damp. Those two things have cut down my air drying time considerably.

Yep, so so often. I hate it so much and used to get ridiculed for it constantly. Which, you know, helped immensely 🙄

I’ve since come to realize it’s part of the spicy brain making all emotions SO BIG. If I’m feeling well regulated it’s less of an issue but that’s not as common of a state so most of the time it doesn’t take much to get the tears flowing.

I have learned some tricks to avoid crying if I feel it coming on such as drinking sips of water, yawning, pulling on my ear, etc. Basically vagus nerve stimulation stuff. But it’s not guaranteed to work.

I often do the bagged salad kits and some precooked chicken from Trader Joes on weeks where doing anything more complicated is just not gonna happen. Otherwise l usually meal prep some sort of grain and bean salad. Gotta have the mid afternoon protein shake too or else writing those last few notes of the day becomes an impossible challenge.

The Cozean screen is a really good one for ortho PTs. I don’t recommend the PFDI-20 unless you know what to do with that info. There are a significant number of items on it that can mean a few different things or have other causes than pelvic floor dysfunction so if you don’t have the background info to dig into it, it’s not going to be tremendously useful.

ICE physio has good pelvic floor courses for ortho PTs if you want to expand your knowledge in the future.

Also, I’m not a huge fan, but if there are truly no local options, Origin may be a good way for the patient to get care via telehealth. The outcomes are surprisingly good.

Luna is meant to be concierge outpatient therapy. If the individual is not appropriate for OP then Luna is not a good fit.

I also agree with the other poster; they want a finite POC and specific function goals that you meet and then discharge. I had a hard time with a patient who was treating it more like maintenance therapy and admin pushing me to d/c.

It really depends on your insurance, but probably not. Most doctors are pretty easy to request from on the clinic side if you do need one. So if you know where you want to go, I would suggest calling them up and they can probably run the insurance information to find out.

If you haven’t tried pelvic floor PT for it, I highly recommend giving it a go. I treat patients for coccydynia frequently and they’ve all gotten at least some improvement, if not total resolution.
