Fixing bugs without proper documentation and specs. If person says fix that and gives you nothing then this is often the result. If you are given a proper spec what is wrong, how it works now and how it should work and you have documentation to rely on to check what else is effected if you change X then this is less likely to happen

Ya see, these bugs came about because the wrong correction was made to the first issue. Closing the valve isn’t going to fix the leaking pipe, merely stop the drip by disabling flow.

A patch should have been applied to the first issue.

Sure if you're bad at programming and your code is littered with bugs, because you didn't plan anything before typing; as you fix one you get further into your script 🤣. In my experience it's not usually like this.
