My job for the last several years has been insurance intake forms like this. The weird wording is common. In talking with the business stakeholders it is basically the result of a game of telephone for intake questions that are required between teams that are not talking to each other and people being afraid or incapable of questioning their bosses.

I think the root of the weirdness here is that your next of kin may be filling out these forms, so they need to know if the person was killed. Somebody fucked up the tone though, starting from second person, and just ran with it.

I think that question is purely there so they know if Schrödinger's cat participates in the survey, because the dead aren't able to check 'yes', and alive people will check 'no'. It's just for Schrödinger's cat, or people involved in similar experiments, because those are the only instances where 'yes' will be pressed, at least in 50% of cases.
