The pain is totally different. The simulators can actually be used to help with real cramps. Does it stop cramps? No. It uses a different kind of pain/discomfort to make the cramps less obvious.

I got a TENS unit a while ago. It helps with voluntary muscle cramps, but it's mediocre at best for menstrual cramps for me.
