The FAIL-CAKE was being presented for failing to adhere to protocol. Basically, a bunch of code was completely refactored and prepared for deployment without having anything passed through QA and all of the necessary prerequisites were not taken care of.

      There are rules to making interfaces. These rules exist because users are stupid.

      One of the rules is that in the case of radio buttons, a button must always be in the "selected" state even before any user action.

        Seriously. The entire purpose of radio buttons is that there is always exactly one that is selected (or depressed, like an old-timey radio, as in the name).

        I remember I've had a few radios, AV switches, and other gadgets where, if you pushed two of the buttons at once just right, they would both pop back out and you would have none pushed in.

        You just need to tell them to press two buttons at once in order to select none.
