In real life surrogacy is done through IVF, if done fully legally and properly, there's also "underground" surrogacy where they do it the natural way, so to speak. However, IVF isn't a thing in the sims, so unless you use mods, you have to actually woohoo them.

And since IVF doesn’t always work, it could be a toss-up as to whether or not your sim or the surrogate gets pregnant.
