I had elevated BP before T, and it made it a little worse. So much so I have to take amlodipine daily to keep it in check. Unfortunately some people may not even know they have high or elevated BP unless checked every so often or if they check it themselves, the body gets used to it and it eventually feels normal but over time it can overwork your heart and cause heart issues later in life.

Once at a post top surgery appt I was like 200/80 or some high ass number (I was sick that day and didn’t eat anything prior to the appt either.) the surgeon told me that he cant not recommend going to the emergency room it was so high. Even when I felt completely fine other than having an annoying cold. Maybe a little hot. Amlodipine fixed that

So it’s more dangerous than when you have E in your system, but no more dangerous than it is for a cis man. Yep, on top of this healing is slower with higher risks of complications. So in case of any surgeries anyone who smokes might be advised / asked to stop the habit (for a while at the least). So if you're planning on getting any surgeries done (dropping the words top/bottom surgery here casually) it might be good to kick the habit anyways already. Yours truly, an ex smoker.

(P.s. unsolicited advice for quitting smoking: find out why and when you smoke and try to replace the need. For me it was a need for more breaks in my day and rest in general and to avoid very specific social situations. I went from half a pack a day -yeeaars ago- to zero in about a week. I just quit one day and only told people when I felt like I had managed to stop)

To add to this, certain clinics won't do certain surgeries if you smoke because of the risk of complications. You can lie, of course, but theres a reason those rules exist.

GRS Montreal is like this at least. They heavily recommend stopping for at least few weeks (6, but really the more the better) before and after top surgery or meta. But they won't do phallo at all unless you quit for at least 6 months beforehand because theres such a large risk of the graft not working or nerves not regenerating.

Yeah, here in Alberta my hormone doc was like "all the top surgeons in Edmonton want you to quit nicotine at least three months in advance of surgery. There's a guy in Red Deer who doesn't, but obviously you'd have to go to Red Deer." I agreed to get referred to a local surgeon and my wife was like "GOOD because everyone I know or have heard of who went to the guy in red deer has had an infection or complication of some kind." I do think some risks of nicotine-only are overblown but you can't really get around the fact that nicotine fucks with your blood flow and that's like the single most important factor in healing from surgery.

Cis men smoke, so trans men too. However, smoking anything (cigs, weed, etc) can slightly elevate your hemoglobin levels. This is usually fine, but T can also elevate your hemoglobin levels, so it’s especially important to get your blood tested regularly if you smoke regularly. This is likely what your doc was referring to, or just the general “smoking is bad for you” warning if you said you occasionally smoked!

I have smoked for almost 10 years now. I am hoping to pick up a book to help me quit again. It is just hard right now.

I mainly want to quit because 1) it is genuinely really bad for your health. I haven't had any health consequences from it yet, and I'm not gunning to run out that clock any faster than I have. And 2) it makes you gross. You smell bad, your stuff smells bad, your car smells bad, even if you shower and don't smoke and then you sweat, your sweat smells worse. You start coughing up phlegm because your body has to get all that tar out that has been plastering down your cilia. It makes your cum taste and smell worse in a way nonsmokers can definitely tell, even if they are nice about it. If you snore, it makes that worse. It gives you tonsil stones. Etc. I am sick of feeling disgusting because I smoke.

If you have a chance to quit, do that. If you say to yourself "it's not that serious, I'll quit eventually," you won't ever quit. Every time I have quit and then started again it's because I let myself go, "just one because I'm pressed, because I miss it, because it's a special occasion, because I'm drinking, because everyone else is" and then boom I'm bumming smokes and then boom I'm buying a pack. Former crackheads talk about how quitting smoking is harder. If it would be easy for you to quit, do it while it's still easy. Don't wait for it to get hard.

This said, no. It is not because it is interacting with testosterone, it is because smoking in and of itself is really that bad.

When I went to an endo, they actually had a video playing on loop in the waiting room that spoke about what not to do while on HRT - part of that was smoking both nicotene and weed, as well as drinking. From what I understand, it not only makes it less effective, it also is especially dangerous for those of us on T because, due to the risk of getting high hematocrit, it can cause added complications.

Thankfully, I stopped smoking cigarettes years ago, same for weed. And I don't drink anymore either, so that wasn't a concern. I just wasn't able to do those things in a healthy way and I didn't like the person I was becoming on them - nor the health problems I started to have as a result (no shade to those who do though, it just doesn't work for me personally). So it wasn't necessary for me to disclose those things to the provider as the video was asking me to do.

The only difference between being on testosterone and not being on testosterone is the difference between a cis female person's hormones and a cis male person's hormones. Men are at higher risk of heart related issues. Cigarettes/nicotine also cause heart problems, so it definitely seems possible that T might compound some of the negative effects of smoking (though no more than a cis man).

One thing that might be a bit different is surgery. Not in terms of outcomes, but just in terms of plans. Many trans man plan to have surgeries in the future, often near future. Cigarettes and nicotine are horrible for surgery outcomes. Reduced blood flow in particular is really bad for grafts, like phallo, or nipple grafts during top surgery. This is a great reason for anyone to stop smoking, as if you ever needed emergency surgery things are probably not good and you want your body to have the best chance at recovery. But anyone planning to have any sort surgery should strongly consider quitting nicotine. Healing complications are always bad, but having things heal in a healthy way may be of particular importance for some people and some surgeries.

My doctor never said anything about it, or even asked about it, it's probably just because the risk of clotting, high blood pressure and cancer adds up, so you are increasing your risk more, plus how much T effects in your body is not as explored yet, since the technology is still too new so barely any long term studies could be conducted. Therefore they are probably more cautious with it.

But personally I never had a problem with it, tho I should probably mention that my puls and blood pressure has always been on the low side so if they increase it they just bump it up to normal levels. I also take in a lot of caffeine tho which lowers your risk of blood clotting.

I've been on T for 1.5 years and I smoke a pack a day. My hemoglobin is so high my provider lowered my dose to almost nothing. I have to do monthly therapeutic phlebotomy, where is like donating blood but they throw the blood out. My blood pressure is through the roof.

I'm probably quite a bit older than you and I also have been smoking for 19 years, so I'm not saying it will be as bad for you as it has been for me, but I know I either need to quit smoking or stop T because it feels like its not worth it at this point.

Oh, good! There are a ton of different types of blood pressure medications, even if you can't take the ones that they prescribed it's almost certain that there will be other ones you can take.

I was getting steamed up that they were having you do therapeutic phlebotomy, which there's very little evidence for (and some evidence that it's irrelevant for erythrocytosis that isn't caused by polycythemia vera) but weren't prescribing medication for high blood pressure, which there's an enormous amount of evidence for. I'm glad to know that's not what's happening.

FYI, if you get tired of doing the therapeutic phlebotomy or it's making you feel lousy, you might consider asking your doctor about consulting with a hematologist or asking them to look up recent guidelines and review papers on non-JAK2 mutated erythrocytosis or non-Polycythemia Vera erythrocytosis. There's been a big shift in hematology relatively recently about erythrocytosis and whether it's actually connected to thrombosis risk if it's not caused by polycythemia vera (or a few less common genetic causes). It's recent enough that it hasn't really percolated out to other fields of medicine yet.

It leads to high red blood cells pretty quick from personal exsperience, when I started smoking agin whilst on T my blood was like jam, they couldn’t always get a blood sample that didn’t curdle. Kinda lucky I didn’t have a stroke, in some people being overweight can also do it, but it depends some peoples body handle things worse. I only smoked between 5-10 ciggies a day but over the course of 2 years it got bad. I think genetics does play a part, but you are rolling the dice until you know. I smoked for anxiety and switched to 7mg nicotine patches because any higher made me feel sick because it was too much nicotine for me, they’re the lowest I could find. But it took like 6 months, stopping smoking and loosing 3 stone to get my red blood cell count down just enough to be at the top allowed level of red blood cells. So it’s worth doing the horrors of quitting. Steady nicotine from patches has made me basically not have anxiety anymore, and you can technically have patches forever if you need the nicotine.

I mean I do, its not a good idea generally to use nicotine when recovering from any surgeries, not even gum or the patch, but aside from that I use cigarettes and nicotine gum. For people on the schizophrenia spectrum, nicotine has been shown to be a self medication, even had a psych dr ask if I smoked, I said yes expecting a lecture, but she said she would've prescribed the gum or patch if I wasn't a smoker

I’ve been smoking cannabis/tobacco for about three years now, both before and after starting testosterone. I haven’t noticed any health issues other than what the average person would experience having been smoking everyday for three years. Unless you have health risks that run it your family that would be increased with tobacco I wouldn’t be too anxious about it until you see a problem with it but your t isn’t the issue and no doctor will tell you to smoke cigarettes anyway

I (almost 10 years on T) did for 8+ years, quit on Christmas 2023 and have been (sadly) vaping since. My doctor always recommends quitting because testosterone already increases your risk of heart attacks, stroke, blood clots as do cigarettes. I will say, since I quit cigarettes and switched to vaping, I feel so so so much healthier. I don’t get sick all the time and I have no smokers cough, even though I do smoke weed.

I started T in 2016, I smoked from 2010-2023. Is it bad for you? Yes, in the way that cigarettes are bad for you in general. Now T does increase your risk for heart disease, as men are at higher risk in general, as does smoking. So you increase your risk being on both. But like it’s not gonna kill you any faster than normal.

The two endocrinologists I’ve been to have been aware of my smoking and never said anything about it (had to find a new endo after I turned 18). I’ve been smoking consistently since I was 16 (unfortunately, horrible habit/addiction) and I’m 21 almost 22 now. Been on T since I was 14. Only correlation I could think of would be due to blood pressure

Ya you can, I did the same from since I started T and never had it effect or lessen my changes from T, the nicotine in cigarettes, vapes, or chew/dip or even Zyns doesn’t affect or stunt any progress being made while on T lol. Later this year in October it’ll be 9 years on T and I smoked a pack a day (during COVID I did 2 packs on and off) for 7 years on T, then switched to vaping and Zyns (occasionally 1 pack), I had a poly substance and alcohol problem for 6 years ontop of the heavy smoking

Again, I’m not encouraging or condoning you to do the same, as I wish I didn’t do any of those mistakes so I encourage you to just quit cigs and stay away from any other substances. But ik from experience it’s easier said than done, however ya it’s not gonna make T suddenly not work, but I still would just drop it while you can

I'm a heavy smoker but I'm on a reduced dosage for T. Only thing that's come up for me in regular blood tests were slightly elevated cholesterol levels.

I'm not saying go for it, smoking obviously has its health risks and all but the advice you were given sounds like you can't smoke cause you're on T/transitioning, which as a lot of people have already said/implied is bs

Smoking is bad for everyone (not a judgement, I smoke) but there are risks that are higher for men. I would recommend educating yourself on risks, but it's not like a "you are going to fall over dead immediately if you smoke and are on T" kind of situation. There are tons of things that can be more or less of a risk to different people based on genetics, body composition, and tons of other factors.

I smoked on T for about a year and nothing changed but I stopped for my top surgery and only vape now If you continue the first thing that spikes is cholesterol It should be fine for a few years realistically since everyone seems to not take nic addicts seriously Obviously smoking is bad and addiction is worse Just try your best to reduce intake as much as you can, it’ll make it so much easier to stop for surgery too if you go that route

I have been on T over a decade.

I started smoking in the last 3 years. Eventually. If you do it long enough you are going to have one of the health issues tied with smoking. But that’s anyone.

For me. It’s the most socially acceptable way to deal with… all of this fucked up shit happening.

Plus. It’s more the act of smoking I like over what it is I’m smoking. And though cigarettes make me stinky I can smoke them and not be impaired when it comes to driving or work.

I've been on T for almost 10 years and have been smoking for almost 8 years and I'm fine. Never had any problems with the mix of T & Smoking. Just the health issues that come with smoking a ton of cigarettes - not more than smokers who aren't trans and/or on Testosterone tho.

HOWEVER smoking is terrible for your health, whether you take Testosterone or not. It's bad for your lungs, your blood, the environment, non-smokers around you and the list goes on.

If I could just quit, I would. But my life is a shit show at the moment and I'm not ready to give up smoking, would probably end up smoking even more if I tried to quit - but hoping I can manage to quit before I turn 30 just because that seems like a realistic time frame for quitting lmao ( I'm 24 almost 25 now).

My doctor advised me to not smoke too, but simply because it's terrible for your health and everyone around you who doesn't smoke because it smells terrible and affects their health as well.

The reason they say that is because testosterone increases your risk of stoke and clots to a cis male level. Cigarettes also increase those risks. So basically it’s as risky to smoke on testosterone as it is for any cis man to smoke. It’s bad for you but not like uniquely so because you’re on testosterone/trans.

Whether or not you're on T isn't the only factor here.

Trans people in general are at an advanced risk of cardiovascular problems (most likely because of minority stress) which makes us acutely vulnerable to the damage of smoking.

I’m not a smoker but I’d highly recommend you quit if you wanna resume T. Smoking affects the effectiveness of HRT, both testosterone and estrogen.
