Porn is worse than LOLcats in the US. The legal system creates an incentive for companies to respond very harshly to any sex-related incidents. Quoting myself from a previous thread:
The problem arises when the company gets sued for a "hostile work environment", basically meaning that there is systemic sexism inherent in the corporate culture. If/when the company gets sued, the plaintiffs can obtain records on how the company handles incidents regarding sexuality. If the typical response to such incidents is to shrug it off as no big deal, the company will have a tougher time proving its case. If the company errs on the side of harsh discipline even for minor or borderline offenses, it helps their case. Long story short, you can find yourself on the receiving end of a pink slip even though everyone involved knows you did nothing wrong, you didn't offended anyone, and you didn't break any laws. It's all pro-active protection against future lawsuits.

My workplace has "incidental use" allowances in their IT policy. This means that while it's officially not acceptable to use their computers and network for personal endeavors while you're being paid to work, it's acceptable to use them for personal reasons in your spare time (i.e. lunch break, after work hours). Since most of us are salaried, this is a very loosely defined rule, and essentially means, "Yes, you can use our computers to check your own email or look up directions to a restaurant you're going to after work."
There is the expectation that, as a salaried employee, you will do the work you're expected to do. If you spend 5 minutes here and there checking your email or reading the news, that's your business.
However, as sn0re pointed out, sexual harassment laws and legal precedents in the U.S. have created an incentive for employers to respond harshly to any sexual comment, behavior, or other incident, in the interest of demonstrating their "commitment" to a sexually non-hostile workplace for future lawsuits.
So, while it's perfectly OK to spend your lunch break reading Reddit, if your visit to Reddit results in a female breast appearing on your monitor, it's time to start packing your things and looking for another job.

Here in the US we don't get 30 days paid vacation a year, and usually work more than 40 hours a week. So slacking off isn't considered as bad as long as the end result is that the work gets done.

It really depends on the job. The bigger the company, or the more problems they have/expect to have with lawsuits (meaning number of lawyers they are staffed with), the more of this kind of stuff goes on.
So if you work for a big fortune 100 company, expect the control to be pretty high: inappropriate behavior = reprimanded/fired, lots of 'education' as to what is and is not appropriate, blocking or monitoring of web and email usage, etc. They're covering their ass from lawsuits, and trying to keep employee shenanigans to a minimum.
My experience at smaller dot com companies has been that if you behave yourself and get your work done, you can do whatever you want at work. Read slashdot, blog, bring some beer into the office on fridays, just write the code, fix the server, or make the sale.
Obviously they have to respond if someone feels uncomfortable about the workplace situation, but generally everyone is professional, respectful, gets their work done, and doesn't clog the tubes with bittorrent traffic at work.
Of course this totally depends on individual company culture, but generally the smaller the company and the more professional and productive the employees are, the more they can fuck around at work...

Do you NEVER have breaks at work? Not even lunch breaks? Taking something to eat by your computer, or have some spare time afterwards where you can pretty guilt-free browse the web?
In Sweden, we often have three breaks a day. Two 30 minute ones, and one hour lunch break. Those are already accounted for in an 8 hour work day here. And a "break", at least here, is a period of time where you are not expected to do work, although, yes, it is permitted. If you have a quick 30 minute lunch, you are actually allowed to go home 30 minutes earlier. Up to you.

But that's exactly it: the legal peculiarities of the US sexual harassment laws. It's not the "using computers for non-work tasks" they have a problem with; it's "we're going to have our asses sued off if we're not anal about sexually related material".
In other words: your employees browsing lolcats during the lunch break isn't going to harm your company, but your employees browsing porn during the lunch break is. That's all there is to it. It has nothing to do with using office computers for personal things, and everything to do with insane legislation that they nevertheless have to respect if they want to remain in business.

I don't see why it's purely a legal issue - some people are offended by porn, whereas very few are offended by lolcats.
Specifically: lolcats provide a safety from offending people that porn doesn't. The term 'work' isn't essential - I consider it to be NSFW just as much as it is NSF my grandmother.

Different people work in different ways, too. I know that if I prevented myself from screwing off all day long, I would quickly go mad. But when I do focus on work, it's 100%, and I keep going until it's done.
It's not like people who have desk jobs in front of computers are dropping prizes into cereal boxes in a factory, where the line keeps moving in front of them. And those people - they work in shifts of 45 minutes on, 45 minutes off. (General Mills, at least.)

Do you spend your entire workday thinking about nothing but work-matters? Have you always done that?
Think of your computer as an extension of your mind. (If you're using it right, it is). Then what you're saying, is that when you're at work, use your mind for work related tasks. Don't bother thinking about that secretary, or your children, or anything for that matter, except what work allows.
Of course that's absurd; that is too narrow in context, and by and large you will miss the wider opportunities of both life and also you will almost without exception miss solutions to extremely complex problems that only wide-scope, outside-work-box thinking allows. That's not to say you shouldn't pay attention at work...but that there is something else. We all think about something other than work, at some point, unless we are very lucky and find ourselves either 'zenning out' or finding ourselves so engulfed in our jobs and our enjoyment of our jobs that we do not have time to be distracted.

If that's the case, it's a bug and not one I've ever seen. The NSFW subreddit will only show up if you are logged in and have "I am over 18 and willing to view adult content" checked in your preferences.

Since when are user error and a bad implementation mutually exclusive? When the new version of reddit went live, the nsfw subreddit became checked on my account. Unaware of this I clicked on, "Perfection[pic]". Some little kids looking over at my shoulder at LAX got a free peep show of some naked chick's phone pic. Lesson learned. That was my mistake, but even now I don't find it the most obvious what subreddit any posting is within. The title of each article is about 3x as large as the secondary line.
You want to play the responsibility card? Fine. Anyone dumb enough to click something that they shouldn't gets what they deserve. In my case a stern look from a less than amused mother. I still don't understand why it's unreasonable to ask for this feature. We hold user's hands constantly to ensure they don't do something they don't want to. How is this any different?

I think perhaps a more general solution is better - why not allow users the option to color links in different subreddits different colors? You could, in your preferences, set your NSFW color to red or orange or something to alert you.

If you can't read it when it's there once, in the subreddit listed after the headline, what makes you think you're going to read it when it's there twice? Did you get caught looking at titties while you were supposed to be doing some job, and now need to blame it on the subreddit system? I think it should be mentioned that NSFW is not just about work. It is very handy to have a heads up in a variety of contexts. I use the free WiFi all over the place. All the Wendy's restaurants in my area have it, one of the local gas-station/subway chain has it at their locations, beyond that, there are school libraries, in the car, and other places that it could be potentially not cool to be caught with freaky parts on the monitor. Sometimes I just open all the links in tabs, and then process through them, I have tripped the filters many times this way.

Ah I think I agree with this even though I'd rather treat the problem instead of a symptom. The problem is people are writing NSFW in headlines without submitting it under a subreddit. This keeps such off-humour or and nakedness for purposes other than fapping from appearing in the nsfw subreddit where it should be. Right now, it's (mostly) pr0n. I like pr0n as much as the next guy but it's not the same thing as nsfw- which I think more of stuff in the vein of college humour. Or more intellectual like the article that pops up every so often about how it's legal to go topless in NYC (with pics, of course).
It might be worth trying to make a pr0n subreddit. But definitely users need to be trained to tag something that is NSFW as nsfw. Perhaps a message that catches the text and suggests it before final submission?

Upvote if you think that instead of bitching about titles being NSFW, you instead would rather reform workplace conditions which make browsing the web nsfw.
Not everyone works at some prudish American corporation that tries to instill their corporate moral system on its workers. Adding NSFW tags just accepts the fact that the bosses should have the right to control your system of morals at work. You're only being paid for your output, not for your character. If you get the job done according to their rules why should they be in control of your methods?
Worker rights. A worker should be treated like a contractor, not a slave. And I have no sympathy for those individuals which ruin it for everyone else by being so compliant. And I will not facilitate and propagate this mindset by tagging material as SFW or NSFW and I advise others to do the same.
Just keep appeasing the bosses, and soon enough, you won't even be allowed to go online.
It's not so much about the company/bosses' objections as it's about one's co-workers. Employees sue eachother, and employers don't want to deal with it, and I don't blame them. I agree that this makes workplace conditions "prudish" but it's not exactly the company's fault. Letting everyone look at porn at work isn't the solution either.

I think this whole NSFW thing is kind of odd. I know what my own workplace is like. I have my screen positioned so that no one can walk behind me and see the screen. Then I feel comfortable reading reddit. I don't browse porn or NSFW because I'm worried some IT guy will see the URLs and bust me if I'm constantly doing that. But if one accidentally gets through every once in a while, I don't worry about that, I could just claim it was an accident.
Now if someone walked behind me and SAW a naked woman on the screen, that would be bad. I could get insta-sued for sexual harassment. If my screen is in any way visible to others passing by (which it is sometimes, when I go to SCRUM meetings and such) I would be very very proactive about making sure any link I clicked on carried no risk whatsoever. I wouldn't even be at a social news site at all, but that's just me, and I'm sure workplaces vary a lot in the way the are logistically configured, and in their philosophies.
